Psychological Assessment Questionnaires

The following psychological questionnaires are used as part of our assessment process. Each questionnaire takes a few minutes to complete, scores itself and provides a short on-screen report showing results and score ranges.

If you are completing these questionnaires as part of a psychological process with GoodCBT.com, you must include your initials in the field provided, so that your clinician can identify your results.

You may also choose to include an email address to receive a copy of your scores and report.

If you complete the questionnaire anonymously, without including initials or an email address, the results will be displayed on screen, but no data will be retained or stored by GoodCBT.com

These questionnaires can be used as self-report assessments and you should always consult a qualified clinician if you are concerned about your results.
